Our Mission
Facilitation & Support For Aging or Remaining In Place With Dignity, Poise, & Grace
We provide advocacy and assistance for individuals of all ages and older adults with long-term physical and cognitive disabilities, and adults with recent acute life-altering conditions. Our goal is to ensure safety in the home environment as an alternative to skilled nursing facility admission when medically safe and appropriate.

Occupational therapy intervention uses everyday life activities (occupations) to promote health, well-being, and your ability to participate in the important activities in your life. This includes any meaningful activity that a person wants to accomplish, including taking care of yourself and your family, working, volunteering, and going to school, among many others." -American Occupational Therapy Association
The Ohio Revised Code defines occupational therapy as “the therapeutic use of everyday activities or occupations to help people participate in roles and situations in various settings with as much functional independence as possible. This includes helping people prepare for daily life despite impairments, activity limitations, or restrictions."
Occupational Therapy Services
Home Consultation Services
Comprehensive home consultations to create simple and complex design schemes for minor environmental adaptations to enhance safety in the home environment. This includes grab bars, ramps, and assistance with DME installation from hospital discharge. Enhancing safety in the home environment while promoting independent engagement in functional ADLs (Activities of Daily Living). We will review use of DME & equipment upon installation to ensure safe and propper use in addition to family and care giver education and support.
Safety Enhancements
Acquisition and installation of safety elements, transfer equipment and DME eg.) grab bars, shower chairs, tub transfer benches, ramps, and additional banisters. Identification and implementation of safest home modifications, including minor adaptations and recommendations to set up first-floor living.
Referral and Coordination
Initiation of onward referrals for major adaptations when needed. Coordination of risk assessments and functional transfer plans for complex moving and handling/ functional transfer requirements to provide to home health aides, care givers, and family members the ability to keep you safe and themselves safe when assisting with functional mobility in the home.
Equipment Recommendations & Acquisition Assistance
Recommendation and assistance with the acquisition of durable medical equipment, grab bars, and other elements to safely navigate the home environment while remaining independent. Education on problem solving techniques and safe functional transfers and functional mobility to support safe participation in ADL in the home. Assistance with navigation of insurance, grants, and funding sources.
Clinical Reasoning and Therapy Framework
Application of clinical reasoning to determine the best occupational therapy framework for service user goals. Assessment and acquisition assistance for complex moving and handling equipment, slings, mobile standing hoists, and gantry hoist systems, and sara steady. Education and demonstration to home health aids, family, and care givers on safe use.
Interdisciplinary Approach
Onward referrals to tissue viability nursing, allied health professionals, and physicians.
Maintaining an interdisciplinary approach to service user-centered care.
Education and Support
Supervision, education, and support for family members, service users, and in-home care staff/ home health aides. Establishing competency to prevent injury and ensure safety during functional transfers and participation ADLs. Demonstration and provision of practical techniques to assist with problem solving in the home environment to enhance independence.
Care Assessment and Reevaluation
Ongoing assessment and reevaluation of care requirements. Recommendations on the level of care needed to ensure safety and maximum independence in the home.
Prevention and Early Intervention
Prevention and early intervention to avoid hospital admission or readmission.
Education and advice on environmental modifications to support remaining/ aging in place. Proactive navigation of practical solutions to aging and/ or remaining in place.
What to expect at your first consultation:
A comprehensive evaluation of you in your home environment that lets us know your history, life experiences, and your interests. We call this your occupational profile, and it tells us what values and activities are important to you during this conversation.
An plan of care that is unique to you to improve your ability to perform daily activities, reach your goals and enjoy life through occupation.
An outcomes evaluation to ensure that the goals set with your occupational therapy practitioner are being met. This will include an evaluation of equipment post installation and your safe and proficient use for ADL in your home.
Note: Occupational therapy practitioners in Ohio are not required to have a referral or prescription to evaluate or treat clients. However, other entities, such as hospital or facility policies, accrediting bodies, and reimbursement agencies, may have different requirements.
Medical Massage Therapy Services
Medical massage therapy as an evidence-based complementary therapy. Massage therapy can reduce pain and anxiety for people with chronic illnesses or cancer, and reduce the physiological load of stress whether chronic, minor, or extreme. It provides complimentary treatment to many conditions including stress-related tension, cancer-related fatigue, sleep disorders, high blood pressure, diabetes, low back pain and depression.

Post Partum Occupational Therapy Services & Support: Specialist Occupational Therapy services to facilitate problem solving in the home and address immediate postnatal barriers to normal daily habits, roles, routines and occupations regardless of natural birth or cesarean section.
Post Breast Cancer Surgery & Reconstruction Occupational Therapy Services & Support: Specialist Occupational Therapy services to support home exercise programs post surgery, and safe phased return to normal occupation post breast cancer surgeries and reconstructive breast surgeries.
TBI & Stroke Occupational Therapy Services & Support: Specialist Occupational Therapy services to support patients of all ages affected by traumatic brain injury & stroke. Providing education and support to patients, family members and care givers in the home.
Organic Home Batch Cooking & Specified Grocery Services
David Barnes is our fabulous chef at Prana Home Health & Wellness. With years of experience cooking as a sous chef for Jean-Robert, David literally brings it all to the table, and with Prana Home Health & Wellness David prepares your meals in your home kitchen to your specification. David also has experience as a private chef for the Lindner family. David spent many years cooking specified diets to the many grateful residents of Marjorie P. Lee Retirement Community. He has experience cooking to dietary specifications and recommendations of a speech-language pathologist and worked closely with them during his tenure at Marjorie P. Lee. David will supply a weekly menu to your dietary requirements and specifications and batch cook twice monthly or to your specification to facilitate healthy meal preparation and alleviate the stress of grocery shopping; especially as and when mobility is a true barrier to accessing the community. David will cook for you in your own home to your specifications or, if necessary, in the event that alternative meal preparation arrangements must be made, can facilitate the use of our offsite commercial kitchen.
Certified Home Yoga Therapy
Tina Walter, C-IAYT is an Integrative Oncology & Wellness Yoga Therapist at a number of Cincinnati and Northern Kentucky hospitals. These include but are not limited to The Christ Hospital, UC Health, and St. Elisabeth's. Tina provides yoga therapy and mind/body interventions while people receive chemotherapy in both inpatient and outpatient clinical spaces. She is well versed in palliative care and end of life services. She teaches group classes for those both in cancer treatment and survivorship at the Cancer Support Community and through the Osher Center at UC Health. Tina has completed extensive trainings to teach yoga asana postures and technique, anatomy, meditation, and pranayama. Tina is an accomplished speaker, published writer, and has presented extensive research on these various topics surrounding holistic health & wellness. As a member of the International Association of Yoga Therapists, Tina enjoys gardening and is a member of the Ault Park Garden Club. A mother of five children, Tina resides in Cincinnati with her husband Bob,-co owner and local coffee roaster of Awakenings Coffee & Wine in Hyde Park.
Community Liaison
Kiana Blakes is our fabulous community liaison and community cook. Kiana will work to support our sustainability efforts in partnership with Kept House to channel a waste stream of durable medical equipment back into the community, especially to those with limited health insurance coverage and financial resources. This equipment will be repurposed and safely reused as appropriate and reviewed by the occupational therapy team at Prana Home Health & Wellness. Kiana also supports our cooking efforts to ensure specifications and menu requests. Kiana brings her vibrant heart forward personality right to you, and if she can not get you exactly what you need on the spot, she will send you in the right direction. Kiana can assist with sign posting you to almost any existing agency and source of funding resources in the local Cincinnati area and beyond to facilitate support. She will bring your personal equipment needs and requirements to our occupational therapy team and our community partners at Kept House who are eager to support returning durable medical equipment back to the community full circle from cradle to grave. Kiana's role and efforts in our organization are essential. Kiana is our primary contact if you should wish to support our sustainability efforts and donate durable medical equipment back to The Greater Cincinnati Community.
About The Owner
Katie Charlton-Perkins, OTR/L, LMT is an experienced specialist occupational therapist and medical massage therapist with international experience in the United Kingdom and the USA. She has practiced as a clinical occupational therapist for two years in Cambridge, United Kingdom, within various complex settings. These clinical settings included early intervention and prevention, rheumatology & hands, neurorehabilitation (stroke & traumatic brain Injury), outpatient mental health & psychosis, and inpatient acute care and rehabilitation. In England, she worked with members of the extremely vulnerable elderly population for the Cambridgeshire County Council, where her cases were highly complex and required synthesized input from multifaceted interdisciplinary teams. She has also provided care in the outpatient rheumatology clinic at Cambridge University Teaching Hospital Addenbrooke's, where management of anxiety and depression as a result of chronic pain was a focal point of the nature of her work, in addition to functional components, e.g., home hand exercise programs, splinting, and education. Within the Cambridgeshire County Council, she worked with individuals with severe physical and cognitive disabilities who were often lacking in the ability to self-advocate as a result, which formed an ultimate barrier to their participation in daily habits, roles, and routines, and society at large. She has worked in several locations, from clinics to working with individuals and their families in their homes to inpatient acute and outpatient clinical settings. Katie has conducted telephone and virtual appointments on behalf of the Cambridge University Hospital Addenbrooke’s Rheumatology Clinic on selected days to support social distancing within our clinic during the height of the COVID-19 pandemic. She gained significant experience in the inpatient and community mental health setting and completed clinicals on an outpatient/ community adult psychosis team. This entailed working with individuals with an array of mental health conditions, from paranoid schizophrenia, bipolar disorder, PTSD, anxiety, depression, and mood disorder. It has always been her utmost duty of care to ensure the safety of all patients, and to increase participation, in addition to signposting individuals and supporting family members to existing and new resources within the community; if not schedule these on behalf of patients upon their request. Much of her work both on behalf of the local authority in Cambridge and for Cambridge University Hospital Addenbrooke’s (NHS) has involved close working with families, social workers, home health support workers, community/ primary care occupational therapists, physicians, and other allied health professionals. She has worked closely with families to support vulnerable individuals; focusing on their patient-centered care and occupational goals. Upon relocation to Cincinnati Ohio, she continues to work for The Christ Hospital in both inpatient acute and inpatient rehabilitation unit settings. In addition; she is also currently employed by Hamilton County Educational Services Center as an occupational therapist working with children with neurodivergent conditions, physical disabilities, autism, and learning difficulties during the academic school year. Katie loves and is very passionate about her work as an occupational therapist within her career trajectory and she would be thrilled to assist you and your family with in-home healthcare and occupational therapy needs.

Kept House, the thoughtful estate sales and transitions company is your one stop shop to downsize and move from your property. Kept house is a local Greater Cincinnati based company that offers total estate transition services. They offer total home liquidation, a sustainable approach to, and assistance with donation to not for profit organizations, and total home clearing services. They also support the initiatives of Prana Home Health & Wellness with a focus on safely recycling durable medical equipment back into the community by donating this equipment to Prana Home Health & Wellness for review by the occupational therapy team in order to benefit the entire community and alleviate financial barriers to low income individuals. Jamie & Greg of Kept House are invaluable partners as a result of their accession to sustainability and not for profit community partnership initiatives. Greg & Jamie are local to Cincinnati and provide Cincinnati's premier and highest quality services in the area of total estate transition services.
Gillian Barns provides support to our team for referrals to and from Kept House, Cincinnati's Premier Estate Sales Company. Gillian is thoughtful and compassionate.
